Symptoms
How you feel, day to day — severity, quality-of-life impact, and urgent flags.
Symptoms are the easiest thing to forget between visits. Log them whenever you remember; even one a day adds up. Each entry has a severity from 0–10, an optional quality-of-life impact, frequency, an 'urgent' flag, and an optional link to a treatment cycle.
What the fields mean
| Symptom name | Free text — 'fatigue', 'neuropathy', 'shortness of breath', 'rash'. |
| Severity (0–10) | Your own scale. Be consistent so trends are readable. |
| Quality-of-life impact | How much it affected today (none / mild / moderate / severe). |
| Frequency | Constant / intermittent / brief, etc. |
| Urgent flag | Tick this when something feels off enough that you'd call your team. |
| Linked treatment cycle | Optional — connects a side effect to which cycle caused it. |
| Started / ended | When the symptom began (and ended, if it has). |

The urgent flag is a way to keep track of things you want to surface at your next visit. It does NOT call anyone. If something is dangerous, call your clinic or your local emergency number.
